You will be asked some general questions about what is happening and why you think you need services.
These questions include:

What is the current problem/issue?

Have you had previous treatment or hospitalizations related to mental health or addiction?

Are you currently using illicit drugs or alcohol?
-
Do you you have any current thoughts of hurting yourself or others?
-
Do you have any medical problems?
-
And other similar questions...
This process takes about ten minutes.
The Pre-Intake Worker will then schedule you for an appointment for an Intake Assessment.
On the day of the Intake Assessment, be prepared to stay from 1½ - 2 hours.
A Clinician will ask you more in-depth questions about the issues that brought you to the agency for services as well as information about your history.
Payment and Schedule of Fees:
UCPC is a private, non-profit service organization. The agency accepts Medicaid, Medicare, and most private insurance plans. We also accept self-pay and accept most forms of payment including cash, credit cards, and personal checks.
For individuals without medical insurance and limited income, the agency provides services based on a sliding scale fee.
This fee is based on an individual's ability to pay.
